Mission of the role (strategic purpose incl. key projects & challenges)

Nutricia Care is a support programme for nursing homes in Ireland. Through this service our ambition is to be the trusted partner for nursing homes, supporting them to optimally manage their patients’ needs by providing a bespoke service offering alongside our product range.   The programme includes the provision of HCP services from Dietitians, Speech & Language Therapists and Tissue Viability Nurses where publicly funded HCPs access is not available. 

 

Key accountabilities, deliverables & KPIs, impact on business results

The Nutricia Care Tissue Viability Nurse will be responsible for ensuring the provision of wound care support to Nutricia Care partner nursing homes on their allocated territory in line with the Nutricia Care Business Strategy including the following:

 

  • To conduct wound assessments and develop care plans for nursing home residents who have been referred to the Nutricia Care Service.
  • To triage all referrals in the allocated territory and ensure that residents are reviewed within a clinically appropriate timeline.
  • Liaise with Nutricia Care TVN Service Lead to allocate referrals to external consultant TVNs as is deemed clinically and commercially appropriate.
  • To undertake thorough record keeping and complete all required activity reports in a timely manner.
  • To develop efficient ways of working including the utilization of innovative resources such as the Nutricia Care telehealth app.
  • To provide technical support and training on wound care to staff and residents in partner nursing homes.
  • Liaise with territory partners to manage relationships with key stakeholders and HCPs in Nutricia Care nursing homes in the region.
  • Collaborate with colleagues to develop support materials and resources for HCPs caring for patients with various types of wounds.
  • To be knowledgeable on the role of Nutrition in the wound healing process, and to be able to provide advice on the correct use of Cubitan as part of a wound care plan (training will be provided).  To liaise with the Nutricia Care Dietetic team to ensure residents at high risk of malnutrition are managed appropriately.
  • Deliver projects as a member of the Nutricia Care TVN and HCP teams to drive innovative clinical solutions to Nutricia Care nursing homes.
  • Meet continuous education requirements.
  • Occasional travel to Head Office for meetings and nationwide travel to national and regional team meetings may be required.

 

Stakeholders

Internal

  • Colleagues within the TVN team responsible for delivering wound care support to Nutricia Care nursing home residents.
  • An active member of the Nutricia Care project team(s), working closely with other field-based colleagues in Sales, Speech & Language Therapy and Dietetic teams as well those involved in coordinating the operations and development of the HCP service.
  • Promote good cross functional relationships with Medical Affairs, Sales, Marketing, Regulatory and Customer Service Teams involved in Nutricia Care service provision and development, and the Adult wound care category.

External

  • TVNs contracted to provide wound care reviews to Nutricia Care nursing homes on behalf of the service.
  • Nursing home residents who are referred to the Nutricia Care wound care service.
  • Nutricia Care customers including Nursing Home healthcare managers (DON/ADONs) and non-HCP managers.
  • All HCPs involved in the nutritional care of nursing home residents – staff nurses, catering, GPs, AHPs.
  • Nursing Home professional bodies and their representatives.

  • Commercial mindset
  • Analytical capabilities, able to decompose a problem and build from ground up and use insights to develop a solution
  • Computer literacy in Microsoft Office
  • Positive, future-oriented perspective, challenging established paradigms and driving for action
  • Growth mindset, sharing and receiving knowledge and looking for personal development beyond existing resources

 

Experience required

  • A qualified nurse registered with the Nursing and Midwifery Board of Ireland (NMBI)
  • Must hold a postgraduate qualification in Wound Management and Tissue Viability
  • Full clean drivers licence
  • Fluent English speaker
