ABOUT THE JOB

We are seeking an experienced, registered nurse to join our healthcare nutrition team as a Souvenaid Patient On-Boarding Nurse. In this permanent role, you will guide patients and caregivers through the Souvenaid Connections on-boarding process, provide evidence-based information to support informed choices, ensure a seamless transition for order placement, and offer ongoing patient support. You will also collaborate with healthcare professionals, monitor key performance indicators, oversee referrals, and develop a comprehensive understanding of commercial practices.

Your responsibilities will include (but are not limited to):

• Patient Onboarding Management: Manage the referral process and assist patients/carers end-to-end.
• Ongoing Patient Support: Provide continuous support to patients /caregivers and collaborate with healthcare professionals.
• Follow-Up Calls: Monitor compliance, drop-offs, clinical assessments, and barriers.
• Record Keeping: Maintain accurate records of calls, samples, follow-ups, and referrals.
• HCP Engagement: Collaborate with healthcare professionals for patient care and timely communication.
• KPI Monitoring: Monitor and achieve key performance indicators, including referral-to-contact metrics
• Commercial Acumen: Develop commercial skills and understanding.
• Performance Review: Regularly review performance against objectives and targets.
• Resource Allocation: Plan and allocate resources for smooth transitions and support.
• Reporting and Collaboration: Present monthly metrics, enhance patient flow, and collaborate cross-functionally.

You excel in creating a welcoming and supportive environment for new patients, ensuring a smooth onboarding process. Your self-motivation and ability to work independently within a national team are key strengths. You enjoy interacting with diverse individuals daily, adeptly handling any challenges that arise. Your adaptability, hands-on approach, and commitment to patient care distinguish you. Passionate about Nutricia brands, you take pride in continuously developing your personal brand both within and outside the organisation.

Ideally, you will have:

• Bachelor's degree in nursing and AHPRA registration.
• Previous experience in geriatric or dementia care (desirable).
• Sales experience (preferred but not essential)
• Knowledge of nursing regulations and guidelines for protecting vulnerable adults.
• Proficiency with EMR systems and patient management software.
• Strong communication, empathy, problem-solving, and organisational abilities.
